Our History

The Community Wellness Partners has been in existence since 1996. Founded as the St. Joseph County Minority Health Coalition (SJCMHC), it has been a grassroots movement of community volunteers, health professionals and organizations concerned about societal disparities and access to equal healthcare for ethnic minorities.


Community Wellness Partners is an affiliate of the Indiana Minority Health Coalition located in Indianapolis and we are one of 15 such coalitions across the state, located in counties with large minority populations.


Our Growth

On March 25, 1966, the revered Dr. Martin Luther King Jr., stated  "Of all the forms of inequality, injustice in health is the most shocking and inhuman."  50 years later, this still holds fast in communities of color across the nation as well as right here in Saint Joseph County, Indiana.  For the past 20 years, Community Wellness Partners has been dedicated to bringing an equality to health injustices, eliminate health disparities and change policies that create healthier communities.


Community Wellness Partners is dedicated to being the voice for communities, both urban and rural, that experience barriers to care like transportation, education and many more.  Community Wellness Partners serves over 6,000 people per year through screenings and referrals, Diabetes education, exercise and movement classes and many other creative ways to motivate permanent change in our everyday lives.
